Our Coalition is committed to connecting residents with the tools and support they need to thrive. This work is guided by our Community Health Needs Assessment (CHNA) for Finney County, a comprehensive study we use to identify local health gaps and prioritize the most pressing issues facing our neighbors.

 

By using the CHNA as our roadmap, we ensure our initiatives—like our Community Resource Guide—are strategically designed to bridge those gaps. This guide brings together trusted local contacts, from health services and family support programs to housing, making it easy to find the resources that fit your specific needs.

For those seeking food assistance, our Food Resource Guide provides up-to-date information on local food pantries, distribution sites, community meals, and nutrition programs across our service area. Together, these tools and the data from our CHNA reflect the coalition’s ongoing effort to ensure every resident has access to reliable support and the opportunity to live well.
